The safety of my clients is the upmost importance to me, as well as doing what is best for everyone. So, before any treatment is completed, each client is required to read and fill out a couple of forms. These documents include a HIPAA Release Form & Client Consent Form. I highly encourage you to ask any questions and share any concerns you may have about your procedure.
